Requesting a transcript

Graduates of Eudora High School may request a transcript at any time. There is no fee for this service. To request a transcript, fill out the Transcript Request Form, and return it to the high school registrar by fax (785-542-4990) or by mail (2203 Church St., P.O. Box 712, Eudora, KS 66025-0712).
