What should I bring to Cardinal Craze?
- Yourself! This event is for ALL Eudora Schools families -- both new enrollees and returning students.
- Checkbook or cash for fee payment and lunch account payment. Sorry, the school district is not set up to accept credit cards at this time. You also will have the option of purchasing spirit apparel, if you desire.
- Parent/guardian driver license or other official form of ID (for notary services)
- Completed forms, if you want to save time. (Blank forms will be available on site, as well.) Click here to learn more about downloading forms.
- Updated vaccination records, if your child has received any vaccinations or boosters since last fall.
- Basic financial records, if you believe your family may be eligible for a fee waiver or the free/reduced lunch program. To complete this application (available for download here), you will need to provide the total gross income from all individuals living in your household. Those not completing an application for waiver will be responsible for paying the entire fee amount.
Student fees and fee waivers
Download a copy of the 2011-2012 fee schedule here. Note that district instructional supply fees will be capped at a maximum of $100 per family. (Out-of-district, all-day kindergarten and driver education fees are not included in the family cap and shall be paid in addition to the cap.) Families with an all-day kindergarten student may choose from a variety of payment plans â by month, quarter, semester or year â during Cardinal Craze.
Families who qualify for the free/reduced lunch program will be eligible for full or partial waivers of student feeds. This eligibility is determined through the Child Nutrition Program; CNP applications will be available at Cardinal Craze or may be downloaded here. To complete this application, you will need to provide the total gross income from all individuals living in your household. Those not completing an application for waiver will be responsible for paying the entire fee amount.
Fee payment methods
At this time, Eudora Schools can accept cash, check or money order. Sorry, credit card or debit card payment is not available.
A Temporary Application for Student Fee Waiver, if used as proof that the appropriate and qualifying financial information has been provided, may be submitted in place of student fee payment. This document will be available at Cardinal Craze. (In order to receive this, it is required to submit the application for the Child Nutrition Program, available for download here.)
All students â both new and returning â must complete paperwork before the start of school. To make it easier and save your valuable time, download your forms ahead of time and fill them out before you arrive at Cardinal Craze.
- Back for another year? Here is a list of the forms returning students, including those students who have gone through Eudora's Kindergarten Roundup, that are most commonly needed for the start of school. Please note that each school may have additional required forms, specific to that building. Those will be available to be picked up and filled out at Cardinal Craze. The following forms are REQUIRED of ALL students returning to Eudora Schools this fall:
- Activity participation and emergency medical treatment consent (free notary services will be available at Cardinal Craze)
- Automated broadcast information
- Shuttle/rural route consent
- Student health information
- Enrollment assistance to families
- EES medication permission (Eudora Elementary students)
- EMS-EHS medication permission (Eudora Middle/Eudora High students)
- KSHSAA concussion form (required for all EMS/EHS students participating in any sport)
- New to Eudora Schools? New students, including kindergartners who did not go through Kindergarten Roundup in April, will need all of the above forms, plus several one-time forms and documents. Enrollment procedures for new students
Students living in district who are new to Eudora Schools can handle enrollment tasks at Cardinal Craze. Non-resident students must pay the out-of-district application fee, and the application must be approved by the building principal, before the enrollment process can formally begin.
