Please use this form for scheduling leave time more than 24 hours in advance.

NOTE: Non-certified staff members who work district-wide, including grounds, maintenance, transportation and others, should contact their supervisor directly to submit a leave request at this time.

Instructions:

1. Fill out the following form completely, taking care to provide all requested information.

2. Hit submit to send your request to your building's head secretary and supervisors for approval.  

3. You may receive further instructions from your supervisor or from someone in the office about scheduling a substitute.

NOTE: If you would like a record of this request, please save a screen shot of the final screen that will appear after you hit submit:

  • On a Mac, hold down these three keys at the same time: command (or open apple) - shift - 3. This key combination will save a screen shot to your desktop.
  • On a PC, click the print article button in your browser. Select File>Print (or Ctrl-P), select PDFCreator and follow the prompts to save as a PDF.

If necessary, you may alternately choose to print a hard copy of the confirmation screen. (Please use an electronic screen shot if possible, to minimize printing costs.)
